This is a qualitative study by Joshua E Richardson and Joan S Ash. (2011) in the Journal of American Medical Informatics Association, entitled “A clinical decision support needs assessment of community-based physicians.” [1]

Introduction

The Clinical Decision Support (CDS) Systems provide healthcare providers and consumers with knowledge and person-based information, intelligently filtered and presented at appropriate times to improve health and healthcare.

CDS supports this knowledge to:

  1. Clinician
  2. Staff
  3. Patients
  4. Other individuals

The goal of the study was to provide a user-centered perspective that could help developers optimize CDS functions to meet the needs of physicians in a community-based setting. [1]

Methods

The purpose of the study was to gain community-based physicians’ perspectives in decision-making, which include:

  1. goals
  2. Environments
  3. Tasks
  4. Desired support tools

The study utilized human-computer interaction model

  1. 30 recorded interviews
  2. 25 Observations - primary care providers
  3. 15 Observations - Urban and rural community based clinics

Conclusion

The findings from the study suggest that the decision support needs to be extended from current process. Two other opinions brought up by the physicians were:

  1. Addition of wide range of clinical decision-making
  2. Improvement in cognitive decision-making

Comments

A well designed CDS system can help clinicians in managing their patients effectively. As and example if CDS can track of routine health maintenance tasks of a provider panel of patients, and then send a reminder or having the ability to generate a list of patients with incomplete recommended health maintenance can help provide better and timely care.
